For service claims the following is needed: Type of service (homemaker, personal, lawn, snow, wood) Who provided the service on what date and description of what was performed How many hours did it take to perform service and what was the hourly rate . Add total hours and total amount at the bottom of the timesheet/bill Signature of provider and beneficiary Mileage claims must include proof of attendance. Appropriate examples of proof are: Visit summary notes from your doctor's visit. A note or letter from the doctor. The note must state that you were seen in their office, the reason for the appointment, and include the appointment date and time. The doctor or doctor's nurse will also need to sign and date the letter. A bill from your insurance company, doctor's office, or the facility. Not Acceptable: Appointment Reminder Card (this only proves an appointment was made, not attended) Co-pay receipts (do not support that the appointment was attended) Mileage is paid to the closest appropriate facility. If you are traveling further than this, proof supporting the distance will need to be submitted with the claim.
